Working in home health often a lot of our patients have recently been discharged from a hospital or skilled nursing facility, placing them at a higher risk of re-hospitalization. When a patient is re-admitted to the hospital there are 3 questions that will help you get all of the information you need in order for your company to keep track of the patient for the purpose of resuming care in a timely manner once the patient is discharged back home.
1. What happened? It's important to know why the patient was admitted to the hospital. Did they fall? Have trouble breathing? Chest pain? The list of potential causes goes on and on… 2. When? You also need to know when the patient went to the hospital. Sometimes you find out that a patient was hospitalized inadvertently when you show up for a scheduled visit. Other times family members will call to notify you. Sometimes you'll have to ask the nurse in an assisted living facility when you find a vacant room. A little detective work here can make a huge difference. 3. Where? Where the patient was admitted will be important for the marketers and other office staff at your company to track the patient's hospital stay. This will allow them to get resumption of care orders once a patient is discharged.
